Looking for Services? Looking To Help A Friend?

LA-HOP is an online service request form that can be used by all community members. It is designed to send out specialized outreach and engagement teams to assist people experiencing homelessness in Hawthorne and LA County. This form can be used for people who are seen living in unfit human living conditions such as vehicles, parks, encampments, freeway underpasses, etc.

Services

Housing/Case Management:
Referrals, assessments to various shelters, interim bride housing programs and assistance to permanent housing.

Snacks & Hygiene Kits: Office distribution and outreach services.

Document Readiness: Assistance with birth-certificates and DMV vouchers

LA CADA: Los Angeles Centers for Alcohol & Drug Abuse
A path to Recovery and Healthy Living. Assistance, navigation and coordination with Detox and Recovery Services.

Counseling: OPEN PATHS 310 258-9677 Services at SMC

Mobile Showers: Mondays 8:30am-1pm at SMC 310 672-2208

Venice Mobile Health Clinic: Mondays at SMC 310 672-2208

Mail Services: Free 2 year contract at SMC 310 672-2208

Immigration: Application Assistance located at SMC 310 672-2208
